What we collect
When you send the form we keep what you type: your name, practice name, role, practice type, email, and optionally your phone and city. We also record your IP address and browser details with your message, to investigate spam or misuse, and we keep them with the record. If you arrived through a link or QR code that carries a referral code or campaign tag, we record that code and tag with your message so we know which introduction brought you here. Your browser keeps that code for up to 90 days on this site only. Nobody is paid for a referral. The form has no free text box, and we never ask for patient information.
How we use it
We use your details to reply to you, to talk about whether working together fits, and to investigate spam. We do not sell your details. We may email you about YesOnUs; reply stop and we will not email you again.
Who handles it
Your details are stored in our own records, which are hosted by service providers that run our systems. A short alert in our team chat shows only the practice name and type, and the referral code if there was one. Our web host keeps standard access logs, including IP addresses.
How long we keep it
We keep each submission as a record of who contacted us. To see or correct your details, write to us and we will add the correction alongside the original. If you ask us to stop contacting you, we will, and we keep a note of that request. Your details are never sold.

Write to providers@yesonus.com.
Fonts
The site loads its fonts from Google, which receives your IP address when the page loads. Because we do no tracking across sites, a Do Not Track signal changes nothing here.
Effective October 1, 2026. If we change this notice we will update this date.
